Job Description
POSITION SUMMARY: The Case Manager coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization; monitors health care resources; and assumes a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of care management such as clinical protocols and the analysis of variance. The Case Manager will perform a wide variety of high level patient care activities to include admission assessment, screen for appropriateness, protocol monitoring, development of a plan, and patient and family teaching. Timely and appropriate discharges will be accomplished through assessment of the patient’s post discharge needs and the provision of services necessary to meet those needs. The Case Manager must be knowledgeable in community resources and alternate care facilities. Interacts with physicians, staff, patient/family and outside agencies. The Case Manager identifies opportunities for improvement in patient care and facilitates patient care support functions provided to neonatal adolescent, adults and geriatric patients.
Position requires some weekend coverage on a rotation basis.
EDUCATION/EXPERIENCE: Graduate of an accredited school of nursing with Georgia RN license. A strong clinical background is required. One year previous experience in case management, utilization review to include knowledge of managed care systems. Demonstrated computer literacy. Knowledge of monitoring and evaluation techniques. Basic understanding of fiscal and project management. Good oral and written communication skills.
